Long-Term Home Health Care Program
Since most people wish to remain at home for as long as possible, we have a program to make that wish come true. Our Long Term Home Health Care Program (LTHHCP), also known as “Nursing Home Without Walls” and the Lombardi Program, provides patients and their families with a popular alternative to long term care. A comprehensive package of coordinated services is designed to meet the specific needs of patients and to provide visits and ongoing care in their homes. The hallmark of this program is case management by a Registered Nurse.

     ○ Transportation
     ○ Laboratory Services

Adult Day Health Care Program
At Cold Spring Hills our Adult Day Health Care Program promotes maximum functioning and health capabilities of clients while contributing to their self-esteem. The program offers:

     ○ Safe, supportive home-like environment
     ○ Open seven (7) days a week from 9:00 a.m. to 3:00 p.m.
     ○ Door-to-door service in handicapped accessible vans
     ○ Individualized health/medical regimes based on need, ability and interest
     ○ Providing comprehensive services to individuals with medical and emotional     
        issues including Alzheimer’s Disease.
     ○ High staff-to-client ratio for greater individualized attention
     ○ Supervised community outings and social events
     ○ Arts and crafts, games, music, entertainment and socialization
     ○ Activities of daily living to encourage self-sufficiency
     ○ Daily hot lunches and snacks
     ○ Physical, occupational and speech therapies as prescribed
     ○ Administration and/or monitoring of medications
     ○ Tube feeding, colostomy care, wound care, suctioning, tracheostomy care
     ○ Assistance with personal care such as bathing, shaving and toileting
     ○ Certified Social Worker for case management and coordination of services
     ○ Dental, Podiatry, Psychiatry, Psychology, Audiology, Ophthalmology and
        Laboratory Services
     ○ Monthly Caregiver Support Group
