Referrals:
A referral requires the completion of a Patient Review Instrument
(PRI) and Screen by a Clinical Evaluator, which is submitted to the
Admissions Department of Cold Spring Hills. Referrals may be made by
a variety of sources including but not limited to:

Planners
○ Community Resource
Organization
○ Another Long Term
Care Facility
Payment Sources:
Medicare
Medicare Managed Care
Medicaid
Self Pay
Private Health Insurance
Workers Compensation
Veterans
Health Maintenance Organizations (HMO’s)

contact our Admissions Staff and they will assist you further.
Medicaid Eligibility Assessment:
We can assist applicants with the submission of a Medicaid
Application. Some applicants choose to use their own representative
for determining Medicaid eligibility.
